Effectiveness of transdiagnostic seminars to support patients with common mental disorders: a multi-service practice research network study

Background:
This was a multi-site evaluation of psycho-educational transdiagnostic seminars (TDS) as a pre-treatment intervention to enhance the effectiveness and utilisation of high-intensity cognitive behavioural therapy (CBT).
Aims:To evaluate the effectiveness of TDS combined with high-intensity CBT (TDS+CBT) versus a matched sample receiving CBT only. Second, to determine the consistency of results across participating services which employed CBT+TDS. Finally, to determine the acceptability of TDS across patients with different psychological disorders.
Method:106 patients across three services voluntarily attended TDS while on a waiting list for CBT (TDS+CBT). Individual and pooled service pre–post treatment effect sizes were calculated using measures of depression, anxiety and functional impairment. Effectiveness and completion rates for TDS+CBT were compared with a propensity score matched sample from an archival dataset of cases who received high-intensity CBT only.
Results:Pre–post treatment effect sizes for TDS+CBT were comparable to the matched sample. Recovery rates were greater for the group receiving TDS; however, this was not statistically significant. Greater improvements were observed during the waiting-list period for patients who had received TDS for depression (d = 0.49 compared with d = 0.07) and anxiety (d = 0.36 compared with d = 0.04).
Conclusions:Overall, this new evidence found a trend for TDS improving symptoms while awaiting CBT across three separate IAPT services. The effectiveness of TDS now warrants further exploration through an appropriately sized randomised control trial.

Summary
Predicting the impact of SDG 6 on forests and forest people requires a balanced understanding of the relationship between forests and water. Notable aspects are that forest cover reduces annual runoff but correlates well with water quality, and that recycling of evapotranspiration from forests is important for downwind precipitation. Within this context a target-by-target review of SDG 6, informed by South American examples, suggests that the Goal is unlikely to exert a major influence on forest cover. Targets 6.1 and 6.2 concerning water and sanitation provision will have relatively little impact on forests except through a demand for hydrological ecosystem services and the use of wastewater in forestry. Within the four water resources targets (6.3–6.6) significant impacts may be limited to water efficiency considerations (Target 6.4) restricting plantations in water-stressed areas and Integrated Water Resources Management (Target 6.5) driving a more integrated view of catchments and their management. The eventual SDG 6 impact will depend on the context of water–forest relationships (illustrated using the Hindu Kush Himalayas as an example), the extent to which SDG 6 will in fact be implemented and alignment of the Goal with forest policies, based on a landscape approach.

How does living in a battleground state during a presidential election affect an individual’s political engagement? We utilize a unique collection of 113 million Facebook status updates to compare users’ political discussion during the 2008 election. “Battleground” state users are significantly more likely to discuss politics in the campaign season than are users in uncompetitive “blackout” states. Posting a political status update—a form of day-to-day engagement with politics—mediates ∼20 percent of the relationship between exposure to political competition and self-reported voter turnout. This paper is among the first to use a massive quantity of social media data to explain the microfoundations of how people think, feel, and act on a daily basis in response to their political environment.

Summary
This chapter uses the notion of techno cultures to suggest some broad perspectives on the historical development of popular music, especially outside the mainstream West, and to look at a set of distinctive music subcultures based around specific uses of extant technologies. New forms of bourgeois song and social dance emerge, together with commercialized versions of traditional musics, all representing forms of bourgeois synthesis. Film culture that centered around Bollywood constitutes a quintessential culture of mechanical reproduction. Indian film culture and film-music culture, which continue to flourish vigorously today, reflect the persistence of the mass-culture mode of production in specific regional centers even in the new millennium. Digital technology had a dramatically democratizing effect on standards and forms of musicianship. The effects of digital technology on modes of dissemination and exchange have been equally dramatic, although similarly concentrated in wealthier and modernized societies or pockets of societies.

Results of Medical Countermeasure Drills Among 72 Cities Readiness Initiative Metropolitan Statistical Areas, 2008-2009

Objective: The Cities Readiness Initiative is a federally funded program designed to assist 72 metropolitan statistical areas (MSAs) in preparing to dispense life-saving medical countermeasures within 48 hours of a public health emergency. Beginning in 2008, the 72 MSAs were required to conduct 3 drills related to the distribution and dispensing of emergency medical countermeasures. The report describes the results of the first year of pilot data for medical countermeasure drills conducted by the MSAs.
Methods: The MSAs were provided templates with key metrics for 5 functional elements critical for a successful dispensing campaign: personnel call down, site activation, facility setup, pick-list generation, and dispensing throughput. Drill submissions were compiled into single data sets for each of the 5 drills. Analyses were conducted to determine whether the measures were comparable across business and non-business hours. Descriptive statistics were computed for each of the key metrics identified in the 5 drills.
Results: Most drills were conducted on Mondays and Wednesdays during business hours (8:00 am-5:00 pm). The median completion time for the personnel call-down drill was 1 hour during business hours (n = 287) and 55 minutes during non-business hours (n = 136). Site-activation drills were completed in a median of 30 minutes during business hours and 5 minutes during non-business hours. Facility setup drills were completed more rapidly during business hours (75 minutes) compared with non-business hours (96 minutes). During business hours, pick lists were generated in a median of 3 minutes compared with 5 minutes during non-business hours. Aggregate results from the dispensing throughput drills demonstrated that the median observed throughput during business hours (60 people/h) was higher than that during non-business hours (43 people/h).
Conclusion: The results of the analyses from this pilot sample of drill submissions provide a baseline for the determination of a national standard in operational capabilities for local jurisdictions to achieve in their planning efforts for a mass dispensing campaign during an emergency.
